A check, money order, or credit card may be used for payment and
must accompany all registrations. Please make checks payable to
Catholic Charities USA Annual Conference. All payments
must be in US funds. We accept Visa, MasterCard, and American Express.
Members and non-members are welcome to participate in the Catholic
Charities USA Annual Conference and Section Meetings. You may include
individual membership dues or section dues with this registration
form. Please indicate on the form what dues you are paying. Reminder:
If you want to join a particular section as a member, you must be
an individual member of Catholic Charities USA. Contact
Moira Bindner, Director of Membership at 703-549-1390,
ext. 145 for information on your membership status.
Annual Conference (September 13-15)
Agencies/groups with four or more participants are eligible for
a $10 discount per person. Each person needs to complete a registration
form, subtract the discount from the total fee for each person,
and submit ALL FORMS TOGETHER with payment. One-day registrations,
spouses, and guests are not eligible for this group discount.
Persons registering for BOTH the annual conference AND a section
meeting are entitled to a $25 discount off the total fee. Each
section its own own fee for its meeting separate from the Annual
Conference. Please complete the registration form, subtract
the discount and submit completed form with payment. One-day conference
registrations, spouses and guests are not eligible for this discount.
Registration fees will be refunded in full, LESS
a $50 processing fee, for written cancellations
postmarked by Friday, September 5, 2003. Refunds
WILL NOT be given for no-shows or cancellations received after this
date. Substitutions are accepted with written notice. NO refund
requests can be taken via telephone and all refunds will be made
after the conference.
